Integrated Team Based Care Manager (ITBC)
Location: Talbert Health Center, Clackamas, OR and Plaza Health Center, SE Portland, OR. Schedule: Monday–Friday 8:30 a.m.–5:00 p.m.
Job Overview
The ITBC Manager provides operational and clinical oversight for three ITBC teams, ensuring effective team functioning and compliance with regulatory and organizational standards.
Essential Responsibilities
Participate in staff meetings and supervision sessions.
Oversee client treatment and collaborate with TBC leadership to set panel goals.
Manage risk and resolve difficult client situations.
Serve as Leader of the Day to handle client calls and crises.
Maintain safety and operational workflows.
Complete required training and perform other duties as assigned.
Compliance
Ensure federal, state, and county regulatory compliance.
Maintain accurate EHR documentation.
Adhere to abuse reporting laws and HIPAA.
Collaborate with Quality Management/Population Health on compliance.
Monitor KPIs and metrics.
Supervisory Oversight
Supervise Administrative Specialist and Care Coordinator.
Conduct individual and group supervisory sessions.
Make hiring decisions for administrative staff.
Assign daily tasks based on client needs.
Participate in on-call rotation.
Support Care Coordinator in monitoring panel dashboard.
Leadership
Collaborate with TBC Director and other leaders to develop best practices.
Ensure effective team meetings and huddles.
Train and coach team members on TBC workflows.
Identify emerging leaders and provide development.
Provide clinical leadership and training/coaching to TBC teams.
Prepare reports for the TBC Director and stakeholders.
Manage budgets for each team.
Qualifications
Education
Master’s Degree in a Behavioral Science field.
Credentialed as a Qualified Mental Health Professional (QMHP) or ability to obtain within 30 days.
Experience
Minimum 3 years supervisory experience and 2 years program management experience.
