Overview
Sentara Norfolk General Hospital is hiring an Inpatient Case Manager RN to work full‑time. The role is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum. The primary focus is patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay (LOS). This position directly supports hospital patients identified in need of comprehensive case management, promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions, and develops and/or implements comprehensive care plans based on assessment and evaluation.

Generous sign‑on bonus up to $20,000 available for qualified candidates.

Qualifications

12 months nursing experience.

Registered Nurse (RN) license.

Basic Life Support (BLS) certification within 90 days.

BSN preferred.

Case Management certification (ACM, CCM, CCCTM, CMAC or CMGT-BC) required within one year of eligibility.

1 year acute care case management experience preferred.

Responsibilities
Provide comprehensive inpatient case management services to patients in a hospital setting, ensuring appropriate transitions to post‑acute or home care. Coordinate and collaborate with interdisciplinary team members, family caregivers, and community resources to plan and monitor care transitions and discharge planning. Implement evidence‑based case management practices, promote utilization review, and monitor and evaluate patient outcomes.
