Nurse Case Manager

Principal Functions and Responsibilities:

  • Provide medical support to patients and health care providers, including education on specific diseases and treatments and disease management;
  • Provide telephone support (including incoming and outgoing maintenance and adherence support calls) as needed;
  • Manage the database and continuous verification of records and entered information;
  • Administer and adhere to case management protocols, including clinical monitoring, data collection, continuous patient follow-up, compliance, follow-up on treatment interruption or discontinuation, adverse event reporting and liaison with primary health care providers;
  • Prepare for and attend specialized training sessions on specific therapies provided by the pharmaceutical team;
  • Work directly with the reimbursement specialist and the physician to obtain funding for their patients' treatments; help find other funding sources if needed;
  • Attend pharmaceutical sales meetings as needed;
  • Attend patient meetings and conferences as needed to educate patients and providers about disease information/products and case management services;
  • Work with patient associations and advocacy groups;
  • Establish and maintain partnerships with the pharmaceutical team;
  • Act as a liaison with health care providers, pharmacies, and infusion clinics as needed to coordinate treatments, medication administration, and resolve issues that arise;
  • Maintain clear communication with the manager, team, and client regarding the management of patient cases;
  • Participate in the training process of new Nurse Case Managers and provide feedback on protocols, databases, processes, and inter-departmental processes;
  • Report adverse events following program procedures;
  • The Nurse Case Manager will also be assigned other duties and tasks as required from time

Experience and Educational Requirements:

  • A diploma in health care or a related field, with a preference for a bachelor's degree;
  • Licensed Nurse (Registered Nurse) with active registration from the College of Nurses in the province of employment is required.
  • Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance, or related fields;
  • Ideal candidates should be nurses with healthcare and case management background in medical/hospital settings, insurance or bio-pharmaceutical industry;
  • Fluency in French and English is required.

The successful candidate will have daily contacts with unilingual English-speaking customers, patients, or peers from cross-functional teams.
