Overview IDD Specialty Care Manager role at Elevance Health . North Carolina residency is required. This field-based, statewide position supports the CFSP Foster Care program, coordinating whole-person health (physical and behavioral) and collaborating with care teams, families, and state agencies.
Location: State of North Carolina across DSS Regions. Travel within your assigned DSS region is required. When not in the field, you will work virtually from home. Hybrid/onsite expectations will be discussed during the hiring process.
Compensation note: $3,500 sign-on bonus.

Responsibilities Engage collaboratively with families, foster parents, and service teams to craft and sustain individualized treatment plans.
Conduct assessments to identify member needs and develop comprehensive care plans addressing identified objectives and goals.
Support member access to high-quality, cost-effective care and modify plans as needed.
Coordinate with internal and external resources to meet identified needs in terms of integrated (physical and behavioral) whole-person care.
Incorporate social determinants of health into care planning and coordinate with state agencies as appropriate.
Work with field care managers and advisors to support complex medical or IDD cases; lead or participate in specialty projects and initiatives.
Provide education, medication reconciliation assistance, and identify community resources; arrange follow-up care.
Facilitate health rounds with clinical professionals to ensure whole-person health.
Train and mentor staff; perform case/chart audits and support departmental compliance.
Serve as a member of the CFSP leadership team.
Minimum Requirements MS/MA in social work, counseling, or related behavioral health field, or nursing degree with a minimum of 3 years of clinical experience in social work/counseling, including complex psychiatric and/or substance abuse treatment; or equivalent combination of education and experience.
Active, current license as RN, LCSW (as applicable by state), LMHC, LPC (as allowed by state law), LMFT, or Clinical Psychologist to practice as a health professional within applicable state laws.
Preferred Skills, Capabilities, and Experiences Experience working with Children, Youth, and Families served by Local Departments of Social Services through Foster Care and Adoptive Assistance programs is strongly preferred.
Knowledge of resources and opportunities for safe community living for populations receiving LTSS, Behavioral Health, therapeutic, and physical health services.
Experience with service delivery coordination, discharge planning, or behavioral health in a managed care setting preferred.
Clinical experience in social work counseling with complex psychiatric/substance abuse or medical disorders preferred.
Certified Case Manager Certification preferred.
Location and Travel Travel within your assigned DSS Region is required. These roles are statewide field-based and require you to interact with patients, members, or providers in person four to five days per week. Location: North Carolina; remote/hybrid options discussed during hiring.
