Riverwood Center is located in beautiful Southwest Michigan with staff at seven sites across Berrien County. We are accredited by the Commission on Accreditation of Rehabilitation Facilities and a member of Southwest Michigan Behavioral Health, Michigan Association of Community Mental Health Boards, and the National Council for Behavioral Health.

Responsibilities:
Provides case management services in the community to individuals with intellectual
and developmental disabilities and their families
Facilitates linking, advocating, coordinating, and monitoring services; to assist consumers
in gaining access to health services, financial assistance, employment, education, social
services, and natural supports
Assesses needs, develops, and monitors Person Centered Plans and documents treatment

Qualifications:
Bachelor in Social Work or Master's Degree in an appropriate mental health discipline
QIDP, one years experience providing treatment to individuals with Intellectual and
Developmental Disabilities
Must have reliable means of transportation for home visits and maintain Safe Driving Permit
