THIS ROLE IS HYBRID THEREFORE MUST RESIDE IN COLUMBUS, OH FOR CONSIDERATION.

• Monday through Friday
• 8:00 AM - 5:00 PM EST
• Pay Rate $25.00-$30.00 (commensurate with experience)

Summary

The Care Manager Specialist is responsible for the care management of members who are enrolled in the Dual Special Needs Plan. These members are usually stratified as low – medium stratification, or those with Social Determents of Care needs. The Care Manager will work in conjunction with the Nurse Care Manager, Care Coordinator, Transition of Care (TOC) Coach, and other members of the Care Team to improve the member’s health outcomes, address social determinants of health and connect members with community-based organizations.

Requirements

• An active and unrestricted LPN license for the state of OH
• 2 years of med-surg/telehealth experience
• Familiar with community resources & services
• Knowledgeable in Microsoft Office Software
• Valid Driver's License and reliable transportation for possible member visits
