Human Services Professional III – Community Support Program Case Manager – Beloit
Under the direction of the Beloit Community Support Program (CSP) supervisor, the role provides comprehensive, person-centered, team-based, assertive community treatment to persons living with severe and persistent mental illness. Responsibilities include psychotherapy, case management, symptom management, psychosocial rehabilitation, vocational supports, and assistance with activities of daily living, as outlined in individualized treatment plans. The candidate will also manage a small caseload, maintain client records, liaise with external providers, and collaborate with treatment teams, families, and agencies.
Responsibilities
Provide individual and group psychotherapy in accordance with DHS 63 and DSM-5, using CBT, MI, DBT, and other evidence‑based approaches.
Manage a caseload of clients with serious and persistent mental illness and/or co‑occurring disorders, developing and implementing treatment plans, and coordinating needed services.
Conduct risk assessments, document mental status and progress, and communicate pertinent information to the multidisciplinary team.
Advocate for clients, facilitate transitions to least restrictive living situations, and, when necessary, attend and testify at court proceedings.
Maintain accurate, timely client records, benefit eligibility, and reporting in compliance with DHS 63, Social Security, Medicaid, and Medicare requirements.
Serve as a consultant for other CSP teams and student interns on therapeutic interventions and goals.
Participate in team meetings, supervision, training, and policy updates; maintain open communication with family, agencies, and Rock County Crisis Intervention.
Address additional duties as assigned by the supervisor.
Qualifications
Master’s Degree in Social Work or related field from an accredited institution.
State of Wisconsin LCSW or LPC license required or obtainable within 3 years of hire.
Minimum of 1,500 hours in a CSP or 3,000 hours working with persons with severe and persistent mental illness.
Preferred – at least 2 years of experience providing therapy to adults.
Valid, unrestricted driver’s license; auto insurance required.
Physical Requirements
Ability to stand, walk, sit; movement during most of the day.
Capability to travel to client homes and navigate stairs.
Work outside in various weather conditions; potential exposure to unpredictable and aggressive individuals.
Willingness to transport clients in personal vehicle; occasional lifting up to 25 lbs.
