Salary $21.50 per hour
General Description Under the supervision of the Senior Housing Supervisor, the Housing Case Manager coordinates care for resident services. The primary responsibilities include education, support, and accountability to ensure residents receive necessary services and that the program complies with all contractual and regulatory obligations.
Essential Duties and Responsibilities Meet regularly with the Senior Housing Supervisor to review file documentation and ensure compliance with funding regulations, agency policy, and procedure.
Conduct client intake, assessments, and develop individualized service plans.
Assist residents in achieving goals, facilitating groups and activities, and developing self‑advocacy skills.
Make referrals to community‑based services and assist residents in accessing and maintaining entitlements/benefits.
Advocate for residents with landlords, providers, and in treatment teams, ensuring continuity of care and addressing tenant disputes.
Maintain accurate case file records and submit files to the Senior Housing Supervisor within required timeframes.
Conduct routine housing inspections, manage delinquency issues such as rent and phone bill payment, and provide timely interventions.
Attend staff meetings, treatment team meetings, and staff development/training seminars.
Ensure all program criteria and eligibility requirements are met and provide linkages to community resources.
Qualifications Associate’s degree in human services or related field preferred.
Minimum of two years’ experience in human services or related field.
Excellent verbal and written communication skills.
Strong organizational and multi‑tasking abilities.
Confidence in safeguarding confidentiality of client records.
Proficiency in PC’s and Microsoft applications.
Valid NYS driver’s license and reliable vehicle.
Knowledge of community resources and funding systems.
Understanding of mandated reporter obligations.
Physical Requirements Ability to manipulate, handle, and control items or equipment (e.g., motor vehicle).
Stand, sit, walk, bend, stoop, kneel, and reach; climb stairs.
Occasionally lift or move objects weighing up to 10 lbs.
Reading, writing, and interpreting written documents.
