Responsibilities Conduct intake assessments to determine eligibility and client needs.
Develop and update individualized service plans using trauma-informed and motivational interviewing approaches.
Provide housing case management, advocacy, and referrals for benefits and services.
Coordinate with medical professionals, outside agencies, and Ryan White Case Managers to ensure continuity of care.
Track and maintain documentation in compliance with grant and program requirements.
Submit invoices, assist with audits, and support program reporting.
Participate in case conferences, training, and continuous quality improvement (CQI) efforts.
Qualifications High School Diploma required; Medical Assistant certification required within 90 days of hire.
Strong interpersonal, communication, and problem-solving skills.
Proficient in Microsoft Office; able to type at least 40 wpm.
Experience in case management, housing support, or HIV-related services preferred.
Physical Requirements Ability to lift up to 40 lbs. and perform standard office tasks.
