Housing Case Manager
Location: 3131 Gulf Freeway, Houston, TX 77003. Working days: Monday – Friday 8:00 a.m. – 5:00 p.m. (Some Saturdays).

Position Type: Regular | Full-Time.

Salary: $50,003.20–$53,040.00 annually (26 pay periods), plus mileage reimbursement.

Overview
Under the supervision of the Housing Options Supervisor, the Housing Case Manager will work with youth ages 18–25 referred to the HAY Center Housing Continuum to support their housing stability and independent living.

Duties & Responsibilities

Provides housing case management to former foster youth, including reviewing lease agreements, conducting home visits, collecting and reviewing rent ledgers, and providing instruction on communication with landlords.

Assists youth in completing and submitting documentation to local housing authorities throughout the housing process.

Completes required case documentation in a case management database and maintains current case files.

Attends and participates in wrap‑around meetings and HAY Center events to provide status updates and coordinate action items.

Participates in life‑skills training and housing readiness workshops, including weekend sessions as needed.

Requirements
Education:

Bachelor’s degree in Psychology, Sociology, Social Work, Criminal Justice, or relevant field.

Experience:

One year of verifiable full‑time paid case‑management experience with youth in challenging circumstances.

Training with personal computers and Microsoft Office.

Licensure:

Reliable transportation with a valid Texas driver’s license and auto liability insurance.

Knowledge, Skills & Abilities:

Excellent verbal and written communication.

Self‑disciplined, self‑motivated, dependable, and able to work independently.

Excellent time‑management and multi‑tasking skills.

Ability to empathize, handle sensitive situations calmly, and maintain high ethical standards.

Ability to establish effective relationships with youth and stakeholders.

CPR certification preferred.

Knowledge of case‑management principles and documentation.

Preferences

Experience with transition‑age youth (17‑25) transitioning into stable housing.

Experience with public housing and housing subsidies.

Experience with transitional needs of youth aging out of foster care.
