HOUSING ACCESS CASE MANAGER : OnPoint is seeking an energetic, organized, committed professional to fill the position of full-time Housing Access Case Manager. As the Housing Access Case Manager for OnPoint, you will be responsible for supporting individuals seeking housing services from OnPoint through phone and in-person contact, focused on providing and obtaining the information necessary to initiate and advance the coordinated entry process, ultimately determining program eligibility.

Salary: starting at $55,181.18 annually - placement above minimum salary is based on experience.

REQUIRED QUALIFICATIONS:
Bachelor’s degree in human services field
A minimum of one year’s relevant work experience in the human services field or a customer service field
PREFERRED QUALIFICATIONS:
Experience conducting screenings, and intakes in social services, medical, or another related field
Professional experience in homeless services
Licensed/Limited Licensed Bachelor of social work (LLBSW/LBSW)
Qualified Mental Health Professional (QMHP)
Certified Alcohol and Drug Counselor (CADC)
GENERAL RESPONSIBILITIES:
Function as the primary point of contact for those seeking housing services through the Coordinated Entry process
Provide an overview of the process and answer any questions individuals may have
Monitor housing phone line calls and housing email inquiries
Complete all required data entry
Engage with individuals to obtain demographic information and an overview of the reason for seeking services
Obtain financial and other information to determine housing program eligibility
Review and complete required consent forms
Provide community-based services at locations most convenient to the individuals seeking services
Initiate and facilitate homeless diversion for those seeking housing services as appropriate
Support individuals to complete access documents and forms and obtain required documents to determine program eligibility
Ensure internal and external care coordination
Facilitate access to behavioral health screening and access when indicated
Complete documentation according to established accreditation, regulatory, funding, and best practice standards
Participate in supervision, team, and other identified meetings
Perform other duties as assigned
