Hospital Registered Nurse Care Manager
We are seeking a dedicated and experienced Hospital Registered Nurse Care Manager to join our team. This pivotal role focuses on managing patient care from initial assessment upon admission to discharge planning, ensuring a seamless transition and continuity of care. This position requires a proficient caregiver with a proactive approach, capable of assessing patient needs and managing comprehensive care plans effectively. Salary: DOE per year
Essential Duties and Responsibilities:
Perform initial comprehensive assessment upon patient admission to determine immediate care requirements.
Provide ongoing patient assessments to identify changing needs and ensure that care plans are current and effective.
Monitor the effectiveness of all ongoing services to guarantee that they meet patient needs and clinical standards.
Track and document progress toward individual healthcare goals to ensure optimal outcomes.
Oversee patient transitions within the hospital and to subsequent care settings, effectively coordinating all discharge planning activities.
Minimum Position Qualifications:
Bachelor of Science in Nursing (BSN) degree required.
Active New York RN license/registration is mandatory.
At least one year of Hospital Care Management experience.
Current certification in Care Management.
PRI Certification required.
