Ennoble Care’s clinicians go to the home of the patient, providing continuum of care for those with chronic conditions and limited mobility. We offer programs including remote patient monitoring, behavioral health management, and chronic care management.
About the Role Ennoble Care is hiring for a Full-time, Hospice Registered Nurse Case Manager who can work within Union / Essex County, NJ. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care to maximize the comfort and health of both patients and their loved ones/caregivers.
Responsibilities Shall be assigned patient caseload by the Patient Care Coordinator or designee
Assume primary responsibility for a patient caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process
Initiate communication with attending physicians, Hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and loved ones/caregivers
Maintain regular communication with the attending physician concerning patient and loved ones/caregivers
Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support
Provide accurate and timely documentation in electronic medical records
Participate in and contribute appropriate information for Interdisciplinary Group meetings
Coordinate physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers or by providing direct care as appropriate
Meet regularly with Hospice nursing staff to review issues, share professional support, and exchange feedback for professional growth
Attend patient care conferences and facilitate discussion of issues from caseload
Supervise and document the NAC every two weeks or as assigned
Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues
Assume the role of associate nurse during on-call time or to fill patient/family needs in the absence of other primary nurses
Provide appropriate support at time of death and during Bereavement
Participate in orientation and in-service training programs for professional staff
Participate in agency and community programs as required to promote professional growth and understanding of hospice care
Maintain familiarity with agency policies and applicable state and federal hospice regulations
Accept other assignments as appropriate
Qualifications Must be a graduate from an accredited School of Nursing
Must be licensed in NJ as a Registered Nurse by the New Jersey State Board of Nursing
One year of RN experience; Hospice experience preferred
Excellent assessment and problem-solving skills
Strong organizational and time management skills; knowledge of case management processes
Must be a licensed driver with an insured automobile in good working order
Strong verbal and written communication skills
Must be fluent in both English and Spanish
