Overview Hospice RN Case Manager | PRN role at Peterson Health. Located in Kerrville, TX. Independent, not-for-profit rural community hospital with a compassionate, patient-centered care and team-oriented culture.
What You Will Do The Hospice RN Case Manager is a professional nurse who coordinates and directs hospice patient care in the home environment (private residence, skilled nursing facility, assisted living, hospital, etc.) based on individual needs. Responsibilities include independent management of the hospice patient population, advanced assessment, teaching, and decision-making; evaluating patients for appropriateness of hospice care and developing the plan of care in collaboration with the provider, caregivers, and Hospice IDG members; providing follow-up by evaluating the effectiveness of the plan of care and monitoring patient and caregiver responses; delivering patient care per provider orders, department policies and guidelines, regulatory requirements, and best practices; collaborating with the provider, patient, caregivers, and Hospice IDG members to meet goals of care in alignment with patient preferences and hospice philosophy; documenting thoroughly and timely in the patient’s medical record; attending and actively participating in IDG and team meetings; and performing on-call responsibilities and holiday staffing.

Required Education Graduate from an accredited Diploma of Nursing Program or Associate’s Degree in Nursing.

Licensure/Registration/Certification Current Texas state license in appropriate discipline

Preferred Licensure/Registration/Certification Hospice and Palliative Care certified; Wound Care Certified; advanced education or certification in area of specialty.
Required Experience Two years’ experience in healthcare setting in respective discipline.
Preferred Experience Medical-Surgical experience; experience in Case Management; supervisory position in healthcare setting; Home Care or Hospice experience.
