Overview Registered Nurse/Case Manager Hospice Care - Plymouth. M-F 8am to 5pm with some weekend rotation. Job Responsibilities
Performs an initial and ongoing comprehensive assessment of the patient’s and family’s physical, psychosocial, environmental, safety and spiritual needs within 5 days of hospice service election, with reassessments at least every 15 days.
Develops an individualized plan of care based on assessment and in collaboration with the patient’s and family’s goals.
Assesses pain and comfort; develops and implements an individualized pain management plan.
Provides holistic, patient/family-centered care in the patient’s residence to improve quality of life.
Consults and collaborates with the interdisciplinary team and other involved agencies.
Supports patient, family, and caregiver in medical decision making and respects spiritual and cultural beliefs.
Supervises LPN and Home Health/Hospice Aides in accordance with Agency policy and regulatory requirements.
Performs admissions as assigned.
Completes an average of 25 visits per week.
Communicates information appropriately and participates in IDG meetings.
Documents problems, nursing assessments, goals, care provided, teaching, and outcomes; completes documentation on the day care is given and for all orders and supervisory visits every 14 days.
Ensures continuity of care through clear verbal and written reports; teaches patient self-care and relevant care to family or staff as appropriate.
Informs patients and families about hospice rights and benefits; serves as a preceptor to other staff.
Adheres to hospice philosophy and regulatory requirements; attends required meetings and completes yearly skills validations.
Performs other duties as assigned.
Qualifications & Conditions
Must pass Drug Screen and Background check.
Pass TB test.
Provide an active auto insurance policy as a licensed driver.
Provide an active driver’s license.
Ability to provide proof of eligibility to work in the United States.
