About the Role
Ennoble Care is hiring a part‑time Hospice Registered Nurse Case Manager for the Gwinnett County and Monroe, GA region and surrounding areas. The RN Case Manager works under the general direction of the Director of Nursing, attending physicians, and the interdisciplinary group to provide skilled nursing assessments, planning, and care that maximize comfort and health for patients and their loved ones/caregivers.

Responsibilities

Receive patient caseload assignment from the Patient Care Coordinator or designee.

Assume primary responsibility for the nursing process: assessment, planning, implementation, and evaluation.

Initiate communication with attending physicians, hospice staff, and other agencies to coordinate optimal care and resources.

Maintain regular communication with the attending physician concerning patient and family needs.

Seek input from team members to support the patient care plan.

Provide accurate, timely documentation in the electronic medical record.

Participate in interdisciplinary group meetings and contribute information.

Coordinate physical care by teaching caregivers or providing direct care as needed.

Meet with hospice nursing staff to review caseload issues and provide professional support.

Attend patient care conferences and facilitate discussion of caseload issues.

Supervise and document the NAC every two weeks or as assigned.

Inform the Patient Care Coordinator of unusual or potentially problematic situations.

Serve as associate nurse during off‑hour times or in the absence of primary nurses.

Provide support at the time of death and during bereavement.

Participate in orientation and in‑service training programs.

Engage in agency and community programs to promote professional growth.

Maintain familiarity with agency policies and state/federal hospice regulations.

Accept other assignments as appropriate.

Qualifications

Current registered nursing license in the State of GA.

1–2 years of professional nursing experience, preferably in hospice care.

Ability to assess and respond to patient and caregiver needs in varied settings.

Skilled in nursing practice and able to cope with caregiver emotional stress.

Strong observational skills and ability to produce comprehensive, accurate reports.

Team player who supports an interdisciplinary team concept.

Valid driver’s license, good driving record, reliable vehicle, and ability to drive year‑round.

Capable of working independently and taking direction within a multidisciplinary team.
