Hospice RN Case Manager (Gwinnett County / Monroe, GA) - Part-Time
We are seeking a dedicated Hospice Registered Nurse Case Manager to serve patients and families throughout the Gwinnett County and Monroe, GA region. This part‑time role focuses on providing skilled nursing assessments, care planning, and support to maximize patient comfort and family well‑being.

About the Role The RN Case Manager works under the guidance of the Director of Nursing and interdisciplinary team to assess, plan, implement, and evaluate patient care. Responsibilities include:
Managing patient caseloads assigned by the Patient Care Coordinator.
Conducting assessments and developing individualized care plans.
Coordinating with physicians, hospice staff, and external agencies to optimize care.
Maintaining regular communication with physicians and other team members.
Documenting clinical care in electronic medical records accurately and promptly.
Participating in interdisciplinary group meetings and care conferences.
Teaching primary caregivers and volunteers about patient care.
Providing support during end‑of‑life care and bereavement.
Completing supervision documentation, including nursing activity checklists.
Assisting cover hours and responding as the associate nurse when requested.
Engaging in hospice orientation, in‑service training, and community outreach.
Qualifications Current RN license in the State of GA.
1–2 years of professional nursing experience, preferably in hospice care.
Strong assessment, documentation, and communication skills.
Ability to support patients and families emotionally.
Team‑player mindset with respect for interdisciplinary collaboration.
Valid driver’s license, reliable vehicle, and clean driving record for travel.
Ability to work independently and follow direction within a multidisciplinary team.
