Schedule: Monday - Friday

Salary: $80,000 - $90,000 / year (offer based on experience)

Qualifications

Associate’s degree, preferred

Registered Nurse license required in the state(s) practicing

Current and valid CPR certification, preferred (required in Georgia)

Minimum one year of experience, preferred (required in California)

Previous experience in hospice, home health, general medical‑surgical or patient‑population specific (e.g., geriatrics, pediatric) nursing

Responsibilities

Conduct comprehensive nursing assessments and develop individualized care plans in coordination with the interdisciplinary team, addressing physical, emotional, social, and spiritual needs.

Maintain timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirements.

Coordinate care activities including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to ensure continuity and quality of care.

Provide education and support to patients and caregivers, and respond to urgent needs during on‑call or admissions assignments.

Supervise LPNs, LVNs, and hospice aides as needed, and participate in interdisciplinary team meetings, patient conferences, and ongoing clinical education.

Support pre‑hospice services by evaluating patient status via phone, telehealth, or in‑person visits and identifying needs for additional care or services.
