Overview
We are hiring part-time and per diem Registered Nurse (RN) Case Managers to join our dynamic care team at Hospice of Western & Central Massachusetts in Worcester County!

This is an on-the-road position that requires the candidate to have insured and reliable transportation. Weekly pay. 8a-4:30p. $48 to $50 an hour.

The RN Case Manager plans, organizes, and directs hospice care to our patients across the region and directs services to meet the needs of the patients and their families/caregivers.

Education & Experience

Must hold an active and unencumbered RN license in the state.

Hospice, home care, or acute care experience preferred.

Duties & Responsibilities

Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.

Assess and evaluate patient status.

Completes, maintains, and submits accurate and relevant clinical notes regarding patient\'s condition and care given.

Communicate with the physician regarding patient needs and report changes in conditions.

Supervise ancillary personnel.

Develop care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions.

Work in cooperation with the family/caregiver and hospice interdisciplinary team to meet the emotional needs of the patient and family/caregiver.

Ensure equipment and other necessary services are available.

Specific Requirements

Must be willing to travel.

Must be able to read, write, speak, and understand the English language.

Must be a licensed driver with an insured automobile in working order.
