Position Summary RN Case Manager (RNCM) is a clinical role whose expertise supports terminally ill patients receiving exceptional end-of-life care. In this role, the RNCM collaborates with an interdisciplinary team to provide hospice services in patients' homes or community settings. Responsibilities include individualized case management, admission facilitation, clinical support, ongoing care, after-hours call, and other skilled nursing functions related to chronic and end-of-life care.
Salary: $83,000-95,000 annually
Responsibilities Succinctly and concisely convey patient assessments with clinical data to a hospice physician who will determine hospice eligibility
Provide direct patient care to terminally ill patients; assess, identify and address physical, psychosocial, safety, and comfort needs
Anticipate, prevent and treat undesirable symptoms or secondary symptoms
Develop, maintain, and manage an individualized care plan that captures evolving needs
Train, instruct and support the patient, family, and caregiver
Document patient problems, nursing assessments, goals, care provided and outcomes in a timely manner
Consult and collaborate respectfully with the interdisciplinary team and others involved in the patients care
Prepare and present information for bi-weekly IDG meetings
Qualifications Graduate of an accredited school of nursing
Current Minnesota RN licensure
Three years of recent nursing experience in acute, long-term care or home care/hospice/public health
Patient care planning and patient teaching skills
Written and verbal communication skills
Critical thinker
Current Minnesota drivers license, insurance, and daily access to a reliable vehicle
