Overview
We are hiring full-time RN Case Managers to join the Hospice Services of Massachusetts team. This position is an on-the-road role covering portions of Middlesex and Essex Counties. Weekly pay.

Responsibilities
Plan, organize, and direct hospice care as an RN Case Manager and coordinate patient care for the assigned caseload.
Supervise hospice aides.
Complete an initial, comprehensive assessment of the patient and family to determine hospice needs, including a full physical assessment and illness history.
Provide professional nursing care using all elements of the nursing process.
Assess and evaluate patient status by writing and initiating plans of care, regular re-evaluation, and revising the plan of care as needed.
Initiate and modify the plan of care as patient status and needs change.
Use health assessment data to determine nursing diagnoses and develop goals-based care plans that include the patient and family in planning.
Initiate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed.
Counsel the patient and family, provide health care instructions, and assist with activities of daily living to support self-sufficiency and comfort care.
Qualifications
Graduate of an accredited school of nursing; BSN preferred.
One to two years of recent acute care experience in an institutional setting, and one to two years in home care.
Current licensure as an RN in Massachusetts and CPR certification.
