As an RN Case Manager, you will provide compassionate nursing care to clients and their families, work with the interdisciplinary team to develop and implement patient‑ and family‑centered plans of care, lead goals‑of‑care discussions, and provide guidance in healthcare decisions to enhance quality of life.

We are Medicare Certified and State Licensed.

Responsibilities

Provide education to patients/family members regarding the process, plan of care, and offer guidance with healthcare decisions.

Consult with the attending physician and Hospice Medical Director concerning changes in patients’ condition requiring order changes and/or changes to the plan of care.

Coordinate hospice care by working with other team members as well as with patients/family members to ensure needs are met.

Conduct initial as well as ongoing comprehensive assessments.

Perform evaluations for hospice care along with informational visits.

Requirements

Missouri or Compact RN license.

Hospice or long‑term nursing experience.

Valid driver’s license and auto liability insurance.
