Requirements:

Current RN license in California

One year of experience preferred

Minimum one year of case management experience preferred

Experience in hospice preferred

Duties include:

Ensuring quality and safe delivery of hospice services

Providing direct hands-on care within scope of practice

Obtaining orders to promote comfort and quality of life

Keeping the Case Manager informed of patient condition changes

Attending daily patient review meetings and bi-monthly interdisciplinary meetings
