Position Overview

We are currently seeking a skilled and empathetic Registered Nurse (RN) to join our team as a Hospice Case Manager. As a Hospice Case Manager RN, you will have the opportunity to provide holistic, patient-centered care, ensuring comfort and dignity for our patients during their end-of-life journey. Collaborating closely with our interdisciplinary group, you will play a vital role in coordinating and delivering comprehensive hospice services in alignment with the highest professional and ethical standards.

Responsibilities

Conduct thorough assessments of patients' physical, emotional, and psychosocial needs to develop personalized care plans.
Coordinate and oversee the implementation of the care plan, ensuring the provision of quality, compassionate, and interdisciplinary care.
Monitor patients' conditions, identify any changes or concerns, and provide timely interventions and support.
Administer medications, provide symptom management, and perform skilled nursing procedures as needed.
Offer guidance and emotional support to patients and their families, addressing their concerns and promoting open communication.
Collaborate with the interdisciplinary team, including physicians, social workers, spiritual counselors, and therapists, to ensure holistic care.
Maintain accurate and up-to-date documentation using electronic medical records (EMR) systems.
Educate patients, families, and caregivers on the hospice philosophy, treatment options, and available resources.
Participate in regular team meetings, case conferences, and ongoing education to enhance clinical knowledge and skills.

Qualifications

Valid and active California Registered Nurse (RN) license.
Minimum of 1 year of experience in or with hospice, palliative care, or a related healthcare setting.
Comprehensive knowledge of end-of-life care, pain management, symptom control, and emotional support.
Strong assessment, critical thinking, and problem-solving skills.
Excellent communication and interpersonal skills, with the ability to empathize and provide compassionate care.
Proficiency in using electronic medical records (EMR) systems.
Reliable transportation and a valid driver's license.
