Overview We are hiring full-time, part-time, and per diem RN Case Managers to join the Hospice of Western & Central Massachusetts team in Berkshire County!
Up to an $8,000 sign-on bonus available. This is an on-the-road position that requires the candidate to travel and have a reliable and insured vehicle. Weekly pay!

Responsibilities Plan, organize, and direct hospice care. The nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Supervise hospice aides as assigned by the Clinical Supervisor.
Coordinate patient care for assigned caseload as Case Manager.
Initiate and revise the plan of care based on patient status and needs, using health assessment data to determine nursing diagnosis.
Provide comprehensive assessments of the patient and family, including physical assessments and histories of current and previous illnesses, and deliver professional nursing care using the nursing process.
Develop care plans with goals based on nursing diagnoses, incorporating palliative nursing actions, and involving the patient and family in planning.
Administer medications and treatments as prescribed by the physician in the plan of care; provide health care instructions as appropriate.
Counsel the patient and family to meet nursing and related needs and support activities of daily living to promote self-sufficiency and comfort care.
Experience & Education Graduate of an accredited school of nursing; bachelor’s degree preferred.
Must possess an active and unencumbered RN license in Massachusetts.
One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care, preferred.
CPR certification.
