RN Case Manager – Austin area
Shift Schedule: Full Time, Monday-Friday, 8:00 AM - 5:00 PM
Coverage: Round Rock, Pflugerville, Hutto, Cedar Park, and Austin areas.

Qualifications Registered Nurse with a valid state license.
One year of RN experience required.
Hospice experience preferred.
Valid driver's license and good driving record required.
Reliable transportation and willingness to drive to patients' locations.
Must be able to lift 50 pounds.
Clear criminal background check, physical and drug screening.
May be required to work outside of regular hours depending on business needs.
What You'll Be Doing Develop detailed Plans of Care for assigned patients ensuring all necessary services are identified and met while working closely with the Interdisciplinary Group, patients, families, and attending physicians.
Coordinate care for patients and families, providing skilled hospice and palliative care for the remainder of the patient's life.
Provide comprehensive nursing evaluations and assessments.
Provide field supervision to LVN's and Home Health Aides assigned to your patients.
File and maintain case management documentation.
Other duties as assigned.
Anticipated pay range: $91,000 - $93,000 USD
