Hospice Registered Nurse Case Manager Hours

• Full-Time | Monday–Friday, 8:00 AM–4:30 PM
• Some on-call required as needed
• Field work in Chesterfield, Tri-Cities, Hopewell

Hospice Registered Nurse Case Manager Compensation

• The pay for this position is $56.14 an hour
• Benefits are available to full-time employees
• A 401(k) with a company match is available for full-time employees.

Hospice Registered Nurse Case Manager Responsibilities

• Perform comprehensive assessments of high-risk members and develop individualized care plans.
• Collaborate with physicians, social workers, and care teams to ensure continuity and quality of care.
• Address both medical and non-medical needs, including arranging transportation, housing, or other support services.
• Coordinate care transitions between facilities and community settings.
• Monitor and evaluate clinical, functional, and fiscal outcomes, maintaining accurate and detailed documentation.
• Identify opportunities for health promotion, illness prevention, and cost-effective care.
• Serve as a patient advocate, ensuring confidentiality and adherence to care standards.

Hospice Registered Nurse Case Manager Requirements

• Active Registered Nurse (RN) license in Virginia or Compact State.
• Hospice or Home Health experience required
