Role Description You will plan, organize, and direct hospice care as an RN Case Manager. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Experience & Education Graduate of an accredited school of nursing, BSN preferred.
One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
Current licensure as an RN in Massachusetts and CPR certification.
Duties & Responsibilities Supervise hospice aides.
Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs, including a complete physical assessment and history of current and previous illness(es).
Provide professional nursing care using all elements of the nursing process.
Assess and evaluate patient status by writing and initiating plans of care, regularly re-evaluating patient and family/caregiver needs, and revising the plan of care as necessary.
Initiate and revise the plan of care as patient status and needs change.
Use health assessment data to determine nursing diagnoses.
Develop care plans with goals based on nursing diagnoses, incorporating patient and family in the planning process.
Initiate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician in the plan of care.
Counsel the patient and family in meeting nursing and related needs and provide health care instructions as appropriate.
Assist the patient with activities of daily living and support self-sufficiency and comfort care.
Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for the assigned caseload.
