Summary

We are seeking a Registered Nurse Case Manager to join our team at Preferred Peaceful Transitions Hospice. In this role, you will be responsible for coordinating and implementing patient care, conducting assessments, and ensuring the delivery of high-quality comfort care. Your expertise will be vital in managing patient cases effectively and supporting their health journey.

Responsibilities

  • Coordinate and manage patient care plans to ensure optimal lifestyle outcomes.
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
  • Collaborate with interdisciplinary teams to provide holistic patient care.
  • Educate patients and families on disease management and preventive health measures.
  • Ensure compliance with healthcare regulations and agency policies.

Requirements

  • Current Registered Nurse (RN) license in the state of practice.
  • Experience in hospice care or case management preferred.
  • Strong knowledge of geriatrics, infectious disease care, and utilization management.
  • Excellent assessment, communication, and organizational skills.
  • Ability to work independently while managing multiple cases effectively.
