Overview St. Mary’s Home Care and Hospice is seeking a full-time Hospice RN Case Manager in Bucks County, PA. We provide compassionate, exceptional care where people are most comfortable: at home. We have a pioneering care model with Home Care Connect, our integrated virtual care program that helps patients avoid preventable ER visits and hospitalizations. It enhances our clinical excellence with advanced remote monitoring technology and 24/7 access to our Virtual Care Center RNs.
Responsibilities Provide professional nursing care to hospice clients as prescribed by the physician and in compliance with applicable licensure, certification requirements, and organization policies.
Take full responsibility for client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes; serve as the case manager for hospice patients.
Collaborate with the physician and other hospice team members to anticipate and manage patient symptoms, and assist in identifying family/caregiver grief/loss issues and implementing the plan of care.
Provide functional support to Hospice Aides and LPN/LVNs as needed.
Case manage a team of hospice patients; participate in weekly Interdisciplinary Team meetings.
Interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms.
Minimum Qualifications Graduate of an approved nursing education program
Licensure as a Registered Nurse in the State of Pennsylvania
2 years of experience as a professional nurse in acute care, home care, or ER (required)
Home care experience (preferred but not required)
EPIC experience (preferred)
