Essential Functions:

Manages primary caseload of Hospice patients per physician's instructions and in compliance with regulations

Must do on-call rotation

Travel to Applewood Village and Pines Village

Implements and updates care plans to address patient's nursing needs

Educates and supports patients, family, and staff on nursing care and patient needs

Collaborates with the Interdisciplinary Team (IDT) to meet all patient and family care requirements

Supervises Licensed Practical Nurses and Hospice Aides when needed

Some on-call responsibilities with additional financial compensation

Qualifications:

Active and valid NJ RN license

Preferred certification: Certified Hospice and Palliative Nurse (CHPN)

Preferred minimum of 2 years experience in hospice or palliative care but willing to train the right person

Ability to collaborate within an interdisciplinary setting

Knowledge and understanding of hospice philosophy and principles of death/dying

Valid driver's license and reliable, insured transportation in accordance with New Jersey state requirements

Skills:

Exceptional communication and interpersonal skills

Robust critical thinking and problem-solving abilities

Capable of working independently and collaboratively in a multidisciplinary team

Proficient in electronic medical record systems and relevant software applications
