Overview We are hiring full-time, part-time, and per diem RN Case Managers to join the Hospice of Western & Central Massachusetts team in Berkshire County. Up to an $8,000 sign-on bonus. This is an on-the-road position that requires travel and a reliable, insured vehicle. Weekly pay.

Responsibilities Supervise hospice aides.
Complete an initial, comprehensive, and ongoing comprehensive assessment of the patient and family to determine hospice needs.
Provide a complete physical assessment and history of current and previous illnesses.
Provide professional nursing care by utilizing all elements of the nursing process.
Assess and evaluate the patient’s status by writing and initiating a plan of care, regularly re-evaluating the patient and family/caregiver needs, and participating in revising the plan of care as necessary.
Initiate the plan of care and make necessary revisions as patient status and needs change.
Use health assessment data to determine nursing diagnosis.
Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions; include the patient and family in the planning process.
Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician in the plan of care.
Counsel the patient and family in meeting nursing and related needs.
Provide health care instructions to the patient as appropriate per assessment and plan.
Assist the patient with activities of daily living and facilitate the patient’s efforts toward self-sufficiency and optional comfort care.
Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for the assigned caseload.
Experience & Education Graduate of an accredited school of nursing; bachelor’s degree preferred.
Must possess an active and unencumbered RN license in Massachusetts.
One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care, preferred.
CPR certification.
