About the Role:
The Hospice RN Case Manager plays a crucial role in providing compassionate and comprehensive care to patients with terminal illnesses and their families. This position is responsible for assessing patient needs, developing individualized care plans, and coordinating services to ensure the highest quality of life for patients in their final stages. The RN Case Manager will work closely with interdisciplinary teams, including physicians, social workers, and chaplains, to deliver holistic care that addresses physical, emotional, and spiritual needs. Additionally, this role involves educating patients and families about the hospice process, available resources, and end-of-life care options. Ultimately, the Hospice RN Case Manager aims to support patients and their loved ones through a challenging time, ensuring dignity and comfort in their final days.
