We are hiring full-time RN Case Managers to join the Hospice Services of Massachusetts team. This is an on-the-road position covering portions of Middlesex and Essex Counties. Weekly pay!

Responsibilities

Plan, organize, and direct hospice care as an RN Case Manager. Build from community resources to plan and direct services to meet the needs of individuals and families in their homes and communities.

Supervise hospice aides.

Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs, including a complete physical assessment and history of current and previous illness(es).

Provide professional nursing care by utilizing all elements of the nursing process.

Assess and evaluate patient status, write and initiate plans of care, regularly re-evaluate needs, and revise plans as necessary.

Initiate the plan of care and make revisions as patient status and needs change.

Use health assessment data to determine nursing diagnoses and develop care plans that include patient and family in planning.

Initiate appropriate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed by the physician.

Counsel the patient and family in meeting nursing and related needs; provide health care instructions as appropriate.

Assist the patient with activities of daily living and support self-sufficiency and comfort care where appropriate.

Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for the assigned caseload.

Experience & Education

Graduate of an accredited school of nursing; BSN preferred.

One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care.

Current licensure as an RN in Massachusetts and CPR certification.
