St. Mary’s Home Care & Hospice is seeking a full-time Hospice RN Case Manager.
Responsibilities Provide professional nursing care to hospice clients as prescribed by the physician and in compliance with the Nurse Practice Act, licensure requirements, and organizational policies.
Fully responsible for client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes.
Serve as the case manager for hospice patients and collaborate with the physician and interdisciplinary team.
Anticipate and manage patient symptoms; identify family/caregiver grief/loss issues; assist in implementing the Hospice plan of care.
Provide functional support to Hospice Aides and LPN/LVNs as needed.
Case-manage a team of hospice patients; participate in weekly Interdisciplinary meetings.
Interact with the primary care physician and/or hospice medical director as needed to manage patient symptoms.
Qualifications Graduate of an approved nursing education program
Licensure as a Registered Nurse in the State of Pennsylvania
2 years experience as a professional nurse in acute care, home care or ER
Home care experience preferred but not required
EPIC experience preferred
