Eden Hospice is hiring a Registered Nurse (RN) Case Manager to provide I planning, coordinating, providing, and documenting care for a caseload of terminally ill patients from day of admission through day of discharge throughoutKing county.

Performs ongoing accurate and comprehensive assessments including physical and psychosocial assessment on hospice patients. Accurately completes Hospice Item Set (HIS).Caseloads will vary from approximately 12-18 patients, depending on issues such as complexity of patient/family needs, geography, and home-based versus facility-based patients;
Obtains necessary physician orders and collaborates with the physician(s) as the patient's needs dictate.
Develops, implements and updates a written plan of care in coordination with other disciplines for each patient.
Assesses patient's condition, initiates plan of care, re-evaluates and updates as necessary.
Guarantees that changes in patients’ clinical status are communicated to other healthcare team members, necessary office staff, and physician (s) as per Agency policy.
Coordinates care planning and delivery with the physician and other healthcare team members and Agency staff to facilitate optimal patient outcomes.
Attends all interdisciplinary group meetings;
Initiates patient care conferences for complex patients whenever needed;
Recommends needed referrals to other disciplines and/or community resources.
Educates patients, their families/caregivers, and other staff as appropriate.
Delivers and/or oversees skilled nursing care appropriate to the patient’s clinical status and needs and in accordance with the standard of care for patient’s age.
Provides clear, concise documentation in medical record as it relates to reimbursement guidelines and Agency policy and procedure.
Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible. Reassesses pain and notifies the physician when pain is not adequately managed on the current medications
Performs aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
The RN Case Manager manages the patient's care from admission through discharge and coordinates with interdisciplinary team on bereavement.
Education/Experience Requirements: Graduate of an accredited nursing program
Current licensed Registered Nurse (RN) in the state of practice
Experience as an RN commensurate with one (1) of the following preferred: a. One (1) year acute care experience within the last two (2) years. b. One (1) year home health care experience within the last two (2) years. c. One (1) year community health or public health experience within the last two (2) years. d. One (1) year hospice experience within the last two (2) years.
Valid driver’s license and reliable transportation
Proof of automobile insurance (MVR checked)
Proof of current BLS certification
Able to pass criminal background check and national sex offender clearance
Must have proof of up to date COVID vaccine, or submit an exemption request to be reviewed/approved prior to hire
