Overview
Otterbein is a not-for-profit, faith-based ministry and a national leader in retirement living communities and long-term care. The Hospice RN Case Manager will help grow our Hospice ministry as part of an interdisciplinary team that includes nurses, chaplain, and social workers. Hospice care focuses on providing peace, comfort, and quality of life for patients with life-limiting illnesses and supporting their families.
Shift
Full-Time 8:00 a.m.–4:30 p.m., Monday–Friday (on‑call and weekend rotation).
Responsibilities
Primary caseload management.
Provide skilled professional nursing care to patients, their families, and caregivers per physician orders and in compliance with Hospice Medicare Conditions of Participation, Ohio hospice licensure laws, and agency policies and procedures.
Qualifications
Bachelor's degree in nursing (preferred) or graduation from an approved nursing program.
Valid Ohio Registered Nurse (RN) license.
2–4 years of direct patient care experience in hospice and case management preferred.
Experience with end‑of‑life diagnosis preferred.
