Hospice Registered Nurse for Admissions & Case Manager
The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Responsibilities

Complete an initial comprehensive assessment of patient and family to determine hospice needs.

Provide physical assessment and history of current and previous illnesses.

Write and initiate plan of care; regularly re-evaluate patient and family/caregiver needs; revise the plan of care as necessary.

Use health assessment data to determine nursing diagnoses and develop a care plan that incorporates palliative nursing actions.

Administer medications and treatments as prescribed within the physician’s plan of care.

Counsel patients and families in meeting nursing and related needs; provide health care instructions appropriate to the assessment and plan.

Assist patients with activities of daily living and facilitate efforts toward self‑sufficiency and optional comfort care.

Act as case manager when assigned by the Clinical Supervisor and coordinate patient care for assigned caseloads.

Complete, maintain and submit accurate clinical notes, including pain/symptom management outcomes.

Communicate with physicians regarding patient needs and report changes; obtain and receive physician orders as required.

Coordinate care plans with community health professionals and teach patients/families self‑care techniques.

Provide a safe environment and assist with continuity of care; attend interdisciplinary group meetings.

Participate in on‑call duties and ensure arrangements for equipment and equipment services.

Supervise ancillary personnel and delegate responsibilities when required.

Maintain and upgrade professional knowledge through continuing education and participation in quality and performance improvement activities.

Qualifications

Graduate of a National League of Nursing accredited school of nursing.

Current licensure in California and current CPR certification.

Minimum of one year of professional nursing experience in the last three years or a baccalaureate degree in nursing from an NLN accredited program with an active RN license; preferred two years’ experience with at least one year in public health, home care, or hospice nursing.

Licensed driver with a valid, insured automobile in good working order.

Self‑directed with minimal supervision; able to work with minimal guidance.

Excellent observation, verbal, written communication, problem‑solving, basic math, and nursing competency skills.

Ability to perform prolonged or considerable walking, standing, lifting, positioning, and transfer of patients as required.
