Hospice Palliative Care Bridge Nurse (LPN)

Key Responsibilities
Taking and processing palliative care referrals.
Providing support to the palliative care nurse practitioner and medical director.
Scheduling visits to support goals of care as ordered by MD.
Coordinating and communicating patient care goals between all providers.
Making calls to present the palliative program, obtaining legal consents, and assessing interdisciplinary needs.
Ensuring communication of assessment and reassessment findings to attending physician.
Completing all documentation accurately, timely, and in accordance with company policy and procedure.
Informing palliative team of all new admissions and discharges.
Coordinating referrals using the referral process.
Serving as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.
Running billing reports and addressing validation holds/billing errors.
Licensure, Certification, and Educational Requirements
Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN) licensure in the state of practice.
A minimum of three (3) years of nursing experience.
Palliative care, hospice, or home health care experience is preferred.
CHPN (Certified Hospice and Palliative Nurse) is preferred.
