Hospice Nurse RN Case Manager $10,000 Bonus

We are hiring immediately for a Full‑time RN Case Manager with a $10,000 sign‑on bonus. Available shifts include Full time or Per Diem. Please specify your preferred shift when applying.

As a non‑profit 501(c)(3), Kline Galland employees are eligible for the Public Service Loan Forgiveness Program, providing relief on student loans.

Key Responsibilities

Conduct comprehensive assessments and develop collaborative care plans with patients and families.

Provide case management, including education, resource coordination, delegation to team members, pain and symptom management, and skilled medical interventions.

Maintain regulatory compliance and coordinate with the broader care team.

Deliver compassionate care, honoring patients and families during this pivotal time.

Qualifications

Registered Nurse (RN) degree from an accredited program.

Current RN license.

Valid driver’s license, auto insurance, and reliable vehicle.

Prior experience in hospice, oncology, or home health is preferred but not required; a passion for hospice care is essential.
