We are seeking a compassionate Hospice Admissions Registered Nurse to join our team as a Hospice Admissions Nurse throughout Harrisonburg, VA. In this role, you will play a crucial part in ensuring a smooth and empathetic transition for patients and their families into our hospice care program. Your responsibilities will include assessing patient eligibility, explaining hospice services, coordinating admissions, and providing emotional support during a challenging time.

A Brief Overview of Responsibilities :
Assessment and Evaluations: Conduct thorough assessments of potential hospice patients to determine their eligibility for hospice care.
Patient and Family Education: Provide information on the admission process and answer any questions or concerns.
Collaborate with physicians and other healthcare professionals to gather necessary medical information.
Explain the hospice philosophy, services, and benefits to patients and their families.
Collaborate with the interdisciplinary team to facilitate the admissions process.
Document all assessments, admission information, care plans, and interventions in a timely and thorough manner.
Other duties as assigned
Qualifications:
Must be a Nurse Practitioner with a current license to practice in the Commonwealth of Virginia
At least one year of hospice experience is preferred but not required.
