Samaritan is looking for a caring and experienced Registered Nurse Case Manager to deliver high-quality, patient-focused Hospice Care. You will manage a group of patients, making sure they receive expert care focused on comfort, managing their pain, and improving their Quality of Life. We give every new team member special one-on-one training when they start.
This role pays an hourly wage of $32.26 to $43.68, depending on your years of experience.
Key Responsibilities
- Check on patients regularly and assess them for comfort, focusing on pain and symptom relief.
- Create and carry out a personal Care Plan tailored to what each patient needs.
- Teach patients and their families about Hospice Nursing and what to expect with their care.
- Work quickly to get the necessary Physician Orders to manage symptoms effectively.
- Plan and organize your home visit schedule efficiently, keeping track of your time and mileage.
- Connect patients with other members of the Interdisciplinary Team (IDT), like social workers, and work with them to meet the patient's full range of needs.
- Provide total, whole-person care that supports the patient's and family's physical, emotional, and social well-being.
Requirements
- Must have an active Registered Nurse (RN) license in New Jersey.
- At least two (2) years of recent work experience as a nurse.
- A Bachelor of Science in Nursing (BSN) is preferred.
- The ability to speak a second language (Bilingual) is a bonus.
