Join Advocate Health Care as a Hospice Nurse-Case Manager to provide comprehensive, compassionate care for terminally ill patients and their families. This full‑time, day‑shift position is located in the Lake Zurich, Barrington, and surrounding areas.

Base Pay Range
$37.50/hr – $56.25/hr

Major Responsibilities

Identify and coordinate patient/family care to support terminally ill patients and families in all hospice settings. Patient/family contacts are at the discretion of the Case Manager and are a minimum every 15 days.

Provide comfort care at end of life and maximize quality of life using teaching, assessment, and intervention skills.

Educate patients and families regarding disease processes, trajectories, hospice philosophies, signs and symptoms of decline, and end of life. Update the primary physician and comply with agency, accreditation, state, and federal requirements.

Continuously review each patient’s caseload to ensure appropriate utilization of services and hospice level of care criteria are met, making referrals to other team members as needed.

Administer medication and treatments as prescribed by the physician.

Lead the interdisciplinary care group (IDG) for each patient/family, develop and revise the patient’s Plan of Care, coordinate services, and ensure consistent patient care.

Document all care activities in the electronic health record per agency/department standards, accreditation, and regulatory guidelines.

Provide ongoing assessment of patient response to treatments, medication, and teaching; make appropriate changes and communicate updates to the IDG.

Supervise and perform Home Health Aide and Licensed Practical Nurse supervisory visits; instruct paraprofessional staff in accordance with regulations.

Facilitate discharge planning and utilize interagency/system resources to ensure continuity of care after discharge.

Maintain OASIS assessments, hospice assessments, and appropriate preventive and rehabilitative nursing procedures.

Licensure, Registration and Certification Required

Registered Nurse license issued by the state in which the team member practices.

Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).

Valid driver’s license issued by the Division of Motor Vehicles.

Education Required
Completion of an accredited or approved program in Nursing.

Experience Required
Typically requires 1–2 years of nursing experience, including home health, hospice, medical/surgical, long‑term care, emergency care/ICU, or oncology.

Knowledge, Skills & Abilities Required

Competent in principles of deformity prevention, normal range of motion, body mechanics, and body alignment.

Excellent awareness and perception skills for assessing patient needs and recognizing changes in health condition.

Strong communication and interpersonal skills.

Excellent critical thinking and decision‑making, with the ability to work independently.

Excellent prioritization and organizational skills.

Physical Requirements and Working Conditions

Lift up to 50 lbs. from floor to waist, 20 lbs. over the head, and carry up to 40 lbs. a reasonable distance.

Push/pull with 30 lbs. of force; perform sliding transfer of 150 lbs. with a second person present.

Work safely with people of various sizes and weight, including those with complex medical conditions.

Move through confined spaces, bend, kneel, squat, and reach over the head for extended periods.

Be exposed to mechanical, electrical, chemical, blood, and body fluid hazards; wear protective clothing and equipment as needed.

Use fine‑hand manipulation skills; operate all equipment necessary to perform the job.

Maintain functional speech, vision, hearing, and touch.
