Overview Hospice Case Manager - Weekends at Trinity Health At Home. Schedule: Friday - Sunday, 8am - 9pm. Work 36 hours a week and be paid for 40 hours a week.
Position provides professional nursing care to hospice clients as prescribed by the physician, and in compliance with licensure and organizational policies. The Hospice RN Case Manager is responsible for client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes, serving as the case manager for hospice patients. Collaboration with the physician and other team members to manage symptoms and support patient/family/caregiver needs is required. Provides functional support to Hospice Aides and LPN/LVNs as needed.
Note: This role includes a 15K sign-on bonus and primary service areas in Washtenaw County.

Responsibilities Inform the client/caregiver about the agency, care/services available, payment responsibilities, and rights; accurately explain/understand Hospice Benefit.
Develop and implement an individualized Plan of Care with realistic, outcome-focused goals to improve quality of life and dignity.
Make referrals to other disciplines and coordinate care to ensure effective, efficient service.
Assess clients using interview, observation, and evaluation; apply nursing judgment and standards to formulate interventions and recommendations.
Report changes in patient condition timely to physician/case manager and obtain orders for changes in the plan of treatment.
Re-evaluate and update the plan of care based on goals and progress.
Assess patient and family learning needs for education on disease process, self-care, end-of-life care, and goal alignment.
Respond to changes in physical, psychological, or spiritual conditions; model teamwork and respect for cultural diversity.
Maintain and enhance knowledge of pain and symptom management using evidence-based protocols.
Prepare patients and families regarding the disease process and provide anticipatory guidance for death and dying.
Document observations, interventions, and evaluations in the patient record per policy, ensuring timely data submission.
Ensure documentation and orders are up-to-date and completed promptly; document collaborative contacts with physicians and IDT/IDG teams.

Qualifications Graduate of an approved nursing program; current RN licensure in the state of practice.
Minimum of one year experience as a professional acute care nurse; home healthcare/hospice experience preferred.
Strong communication and interpersonal skills with emphasis on grief/loss management.
Must have a current driver's license and reliable transportation.
