Hospice of Southern Maine is seeking full-time RN Case Managers to work in our Home Program. The position is 40-hours, MondayFriday 8:00 a.m.4:30 p.m., one weekend day every 4 weeks and no on-call, providing work/life balance. The RN Case Manager administers and coordinates high-quality, cost-effective hospice care for home care patients within an interdisciplinary team model and oversees the interdisciplinary hospice team to ensure patients physical, emotional and spiritual needs are met.
Sign-on Bonus : $2,000 with the first paycheck and $2,000 after successful completion of 6 months.
Responsibilities
Administer and coordinate high-quality, cost-effective hospice care for home care patients within an interdisciplinary team model.
Oversee the interdisciplinary hospice team to ensure that patients physical, emotional and spiritual needs are met.
Qualifications Graduate of an NLN accredited nursing program; BSN or CHPN preferred.

Home care, hospice and/or oncology nursing experience preferred.
Case management experience preferred.
Solution driven, creative and resourceful problem-solving skills required.
Registered Nurse licensure by the State of Maine
Valid drivers license with an automobile that is insured and in good working order.
BLS certification or the ability to obtain certification within the first 90 days of employment. Certification must be kept active during employment.
