Patient Care Home Health in Rocky Hill, CT, is hiring for a Home Health RN Patient Care Manager !
Prior home health experience required for this clinical operations role
The Home Health Patient Care Manager is responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to ensure the continuity of high-quality care to home health patients, in accordance with physician-prescribed plans of care, and all applicable state and federal laws and regulations.
Responsibilities
Receive referrals, assign clinicians and therapists, and authorize start of care visits.
Coordinate patient home health benefits, medical necessity, and insurance approvals.
Assess patient needs continually, ensure care is individualized, appropriate, and meets physician orders and eligibility criteria.
Develop, implement, and update patient care plans as needed.
Manage communication with physicians, clinicians, patients, and other healthcare providers, documenting interactions appropriately.
Review assessments and care plans daily, providing recommendations to clinicians.
Coordinate care with all disciplines, providers, and caregivers.
Follow up on tests, physician contact, and changes in patient condition, ensuring proper documentation and communication.
Facilitate case conferences and reports, especially in cases of adverse findings or urgent updates.
Assist in patient transfers and discharges.
Receive reports from field clinicians and process new orders and updates.
Ensure timely and accurate documentation of clinical activities and billing.
Review clinician notes weekly, address issues, and escalate trends to the clinical director.
Supervise service delivery, conduct audits, and participate in quality improvement programs.
Enter incidents into the Risk Management system as required.
Assist in onboarding new personnel and provide leadership to the clinical team.
Provide direct patient care when necessary, within scope of practice.
Participate in QAPI programs and on-call rotations.
Complete required training and participate in process management.
Perform other duties as assigned.
Education and Experience
Current RN licensure in the state of practice.
Current CPR certification.
Valid driver's license, vehicle insurance, and access to transportation.
