Home Health RN Case Manager (Part-Time)

We are seeking an experienced Home Health RN Case Manager to join our team. This role is responsible for coordinating patient care, ensuring regulatory compliance, and delivering skilled nursing services in a home health setting. Home Health experience is required.

Key Responsibilities:

• Patient Assessment:
  Conduct comprehensive assessments of patients’ physical, psychosocial, and environmental needs to develop individualized care plans.
• Care Coordination:
  Collaborate with physicians, nurses, and social workers to implement and adjust care plans.
• Documentation:
  Maintain accurate, timely records of patient care, including progress notes and treatment plans, in compliance with healthcare regulations.
• Patient Education:
  Educate patients and families on disease management, medication usage, and rehabilitation strategies.
• Supervision:
  Oversee nursing and support staff, ensuring quality care delivery and adherence to protocols.
• Compliance:
  Stay up to date and ensure compliance with all federal, state, and local home health care regulations.
• On-Call Duties:
  Participate in on-call rotations for clinical Q/A responsibilities as needed.

Position Details:

• Rate: $40 to $45 per hour
