Home Health Physical Therapy Case Manager
Aurora Health Care is looking for a Home Health Physical Therapy Case Manager. This role involves coordinating and delivering Physical Therapy services in home health settings, with a focus on quality of care, continuity of services, and cost-effectiveness.
Overview

Responsibilities
Arrange necessary health-care services for patients requiring Physical Therapy services.
Evaluate and implement appropriate treatment plans for referred patients; emphasize quality of care, continuity of services, and cost-effectiveness.
Provide medically prescribed physical therapy in accordance with policies and professional standards; act as a resource for employees and students.
Conduct physical therapy evaluations, establish plans of care and treatment goals, interpret physician referrals, and perform musculoskeletal, neuromuscular, cardiopulmonary, and/or integument evaluations.
Evaluate outcomes within appropriate time frames; provide ongoing assessment of patient response to treatments and revise plans as indicated.
Maintain and update clinical and patient records according to state and federal guidelines; communicate status reports to physicians regularly.
Manage caseload including scheduling and informing site leadership of coverage needs; manage patient medical conditions and medications; advocate for patients to achieve optimal outcomes.
Review caseload for LUPA, PPS, OBQI outcomes, and determine appropriate number of visits based on progress.
May supervise a caseload of Physical Therapist Assistants (PTAs) and ensure compliance with state regulations.
Perform Home Health Aide joint visits as appropriate; educate patients and caregivers and plan for adaptive devices as needed.
Participate in chart audits and quality improvement activities; assist with discharge planning and coordinate community resources for continuity of care after discharge.
Serve as a resource to employees; provide training, mentoring, and guidance on complex issues; may serve as an instructor for Clinical Student Program.
Perform patient positioning and transporting duties with proper technique as required.
Schedule and Benefits
Scheduled Hours : Mon-Fri, 8:00 am to 4:30 pm, with some weekend rotation requirements.
Sign-on Bonus : $10,000 for external hires.
Licenses & Certifications
Physical Therapist license issued by the state of Wisconsin.
Basic Life Support (BLS) for Healthcare Providers certification (AHA) to be obtained within 6 months unless exempt.
Valid Wisconsin driver’s license.
Education
Bachelor's Degree in Physical Therapy.
Required Experience
Typically requires 3 years of physical therapy rehabilitation experience within an acute hospital, home health care, or other relevant environment.
Knowledge, Skills & Abilities
Knowledge of indications and contraindications of modalities and exercises; proficient with modalities, equipment, and devices used in the work area.
Ability to work independently and make sound judgments; strong human relations, organizational, problem-solving, and decision-making skills.
Basic proficiency in Microsoft Office (Excel, PowerPoint, Word) or similar products.
Must possess a valid Driver License and have a registered, functional car with adequate insurance.
