Home Care Coordinator

What You’ll Do as a Care Coordinator:

As part of our care team, you’ll help clients remain safe, comfortable, and independent at home:

Scheduling & Staffing

  • Coordinate daily and weekly caregiver schedules, ensuring all shifts are filled promptly and accurately.
  • Match caregivers to clients based on skills, experience, location, and preferences.
  • Monitor open shifts and proactively resolve coverage gaps.
  • Maintain real-time updates in the agency’s scheduling system

Client & Caregiver Communication

  • Serve as the main contact for clients and caregivers regarding schedules and service needs.
  • Handle attendance issues, call-outs, and last-minute changes professionally and efficiently.
  • Foster strong relationships to support caregiver retention and client satisfaction.
  • Communicate client status updates or concerns to the nursing or supervisory team promptly.

Compliance & Documentation

  • Schedule only compliant caregivers with valid credentials, background checks, and in-services.
  • Keep caregiver availability, assignments, and attendance records accurate and current.
  • Partner with HR and clinical teams to track and maintain caregiver compliance requirements.
  • Oversee and support Certified Home Health Aides (CHHAs) and other support staff.

 

What You’ll Need:

  • Ability to speak, read and write in Spanish (Bilingual)
  • Minimum 1–2 years of experience in home care staffing, scheduling, or healthcare coordination.
  • Familiarity with electronic scheduling systems and EVV platforms. (HHAeXchange, AlayaCare, etc)
