Home Care Coordinator – All Connecticut Home Care Services
All Connecticut Home Care Services is seeking a compassionate, motivated, and organized Home Care Coordinator to join our fast-growing team. This role is ideal for someone with strong communication skills and a passion for serving the disabled and elderly communities. The ideal candidate will be experienced in client intake, staff supervision, and coordination of care, with a working knowledge of Medicaid and private pay systems.
Key Responsibilities:
Serve as the first point of contact for Medicaid referrals and client intake.
Coordinate and maintain client files (Medicaid and private pay) in full compliance with agency policies and state regulations.
Recruit, interview, and hire home health aides, including CNAs and PCAs.
Supervise caregivers and ensure high standards of client care and satisfaction.
Monitor staffing needs and conduct background checks on potential employees.
Maintain accurate and up-to-date electronic records.
Conduct client outreach and ensure ongoing satisfaction with services provided.
Support marketing efforts to identify and grow private client base, including outreach to retirement centers, veteran homes, and other potential referral sources.
